one of the most important advances that dr.tebbetts has made in the specialty of breast augmentation is the developing and publishingthe high five system of measurement. the high five system has over 15 years ofexperience behind it and it allows us to determine the size of your new breast without all theguess work. we plan operations by the numbers. let's see how this works. so we're just going to do the basic measurementsfor the high five system. there are two pinch thickness measurements. that's 11mm, 10mm.numbers don't mean anything to you right now. the base width of the breast. 12-1/2cm. thenipple to fold distance on the breast. 9cm. you're going to feel a lot of tension on yourskin. it's not uncomfortable, just weird.

that's maximum stretch, if you lift up yourskin it won't stretch any further than that. so i'm holding my finger at that point, imeasure back to the breast. that's the stretch measurement and that's what the breast accommodatessafely. this is our clinical evaluation and operativeplanning sheet where the high five system allows us to take the patient measurements,record them, and then let the system calculate the appropriate implant size precisely tofit the dimensions of that patient's skin envelope. on this particular patient, thesoft tissue pinch thickness was 1.1cm of the upper pole. if that number is less than 2cmwe're going to recommend partial muscle coverage so that you, in a lifetime, will never seethe upper edge of an implant or see rippling

in that area. the soft tissue pinch thicknessat the infra-mammary fold was 1.0cm. if that's exceedingly thin, less than 4mm, we recommendthat folks simply don't have an augmentation. the base width of the breast was 12.5cm, andwe take that base width and we come over to 12.5cm, and it tells us that the appropriatestarting volume for that is 300cc's and so we record that in the far right hand column.we know that we're going to do a plain muscle coverage because of the thin pinch thickness. next we usually measure the nipple to folddistance and in this patient, it was 9cm. if it's greater than 9-1/2cm, that means thatthere's more skin in the lower part of the bag that we have to fill so we have to addvolume. but at this case, at 9cm, it has no

effect. next we're going to measure the skin stretch.you saw us pull the skin forward and measure the skin stretch. in this patient, it was2.9. if that's greater than 3cm, we have to add some volume because the stretchier theenvelope, the larger the bag and the more volume required to fill it. but in this caseit had no effect. and then finally we estimate the amount ofbreast tissue that's already in the breast relative to the stretched skin envelope. inother words, with how much of the stretched skin envelope would the current amount ofbreast tissue fill? and in this patient, it's about 65% and that has no effect as well.so the entire volume the system recommends

for this patient is 300cc's. so now that you've seen the measurements,i have a question for you. what is a full c? or a small d? i guarantee you, whateveryou think it is, your friend, your sister, certainly your surgeon are going to thinkit's something entirely different. when subjective methods like bra cup sizing,stuffing sizers in a bra, the rice test, or computer imaging are used to plan your operation,let the guesswork begin. instead of depending on things that we can't define to plan youroperation, why not choose a scientifically proven system like high five? then you'llnever have to wonder if we understood what you meant by full c.
